Supervised Visitation and Guardian ad Litem Services 

SUPERVISED VISITATION FEES

Monday - Friday $30 an hour plus travel time
Saturday - Sunday $40 an hour plus travel time 
Holidays - $50 an hour plus travel time 

(Holidays: New Year’s Day and the 2nd, Martin Luther King’s Birthday, President's Day, Valentine’s Day, St. Patrick’s Day, Easter Weekend, Mother’s Day, Memorial Day Weekend - 3 days, Father’s Day, Independence Day, Labor Day Weekend - 3 days, Columbus Day, Halloween, Veterans Day, Thanksgiving Weekend - 4 days, Hanukkah, Christmas Eve, Christmas Day and the 26th, New Years Eve)

Travel Time: $25.20 an hour billed from office to visitation site and back to office.

SUPERVISED VISITATION HOURS

Monday - Sunday:  9:00am - 7:00pm
All Holidays:            9:00am - 9:00pm 

Maximum visitation time is 2 hours.  Four (4) hours may be approved for out-of-state visiting clients. 

Cancellation without 24 hours Notice: $50 flat fee (no charge if cancelled due to extreme weather).

OTHER FEES

Supervisor's Observation Reports: 
   $15 per exchange & visitation summary 
   $35 per 1-2 hour visitation & supervised telephone calls
   $50 per 3-4 hour visitation

Court Reports/Declarations: $50
Photocopies: .10 per page
Court Appearances: $100 per day + travel time

Letters: $25
NSF Check: $35
Postage: As Required

E-Mails To/From Office: No Charge
Incoming Fax: .25 per page
Outgoing Fax: $1.00 first page/.25 each additional page


SUPERVISED EXCHANGE FEES

Flat Fee: $25 + travel time.
Travel Time: $25.20 an hour billed from office to exchange site and back to office.


SUPERVISED TELEPHONE CALLS

Flat Fee: $25 per call.  Maximum call time is 1 hour.


INTAKE FEE

$25 per client (one-time fee)  Must be paid at time Guidelines are returned.  Intake fee is non-refundable.

OTHER VISITATION COSTS

Person paying for visitation is responsible for supervisor's cost incurred during visitation, (movies, games, entertainment, gate fees, tickets, etc.)  Supervisor is responsible for own meals.

Person paying for visitation is responsible for additional mileage costs if supervisor provides transportation during visit.  Mileage is billed at .60 per mile. 

If visit/exchange is terminated during the visitation for any reason, person paying for visitation is responsible for scheduled fees due and agreed upon prior to visit. 

Weekend scheduled visitation MUST BE CONFIRMED by 12:00 p.m. the Friday before.  Any weekend visits not confirmed will be cancelled.   

Visits/Exchange fees may be paid in advance, however, all fees must be paid at the start of each service.  Payment may be made by cash, money order, or personal check.  

All fees are billed once a month and must be paid in full within fourteen (14) days of invoice date.
